To avoid delays in the admissions process, please begin gathering the following required documents. If you do not already have them, start the process of requesting them as soon as possible:

• Augusta Housing Authority form
• Tuberculosis (TB or PPD) test results
• Syphilis (RPR) test results
• Income verification (WG-15 from the Department of Labor, SSI, SSDI, etc.)
• Government-issued photo ID (Driver’s License or State ID)
• Social Security documentation
• Homeless verification (if applicable)

Applications submitted after 3:00 PM on Thursday will be reviewed the following Monday.
