Licensed Practical Nurse (LPN) - Weekend Shift

 

OVERNIGHT COUNSELOR ASSISTANT

Shift Requirements
Friday - Sunday 7:00am to 8:30pm

Pay Range
$22-$26 hourly
Pay rate depends on education and experience

Job Summary
The LPN at Hope House provides direct support to the Medical Team at Hope House and reports to the Medical Coordinator. This is a hands-on and participative team member who supports day-to-day operations within the medical department including providing care to participants and their children, assisting with medications, completing drug screens, and administrative support where required.

Job Details

Essential Functions

  • Documents participant records within EMR system.

  • Performs in-office drug screens for program participants and staff as needed.

  • Performs TB skin tests and readings as needed.

  • Assists with participant medication management ensuring medications are recorded, stored, and distributed appropriately.

  • Assists assigned doctors as needed.

  • Completes nursing assessments for new participants upon admission.

  • Maintains patient and personal safety regarding occupational health and safety.

  • Supervises the self-administration of medication by program participant in accordance with physician’s prescription order.

  • Administers first aid care when necessary.

  • Maintains appropriate medical case files in accord with legal requirements (i.e. HUD, DHR, and DCA).

  • Displays a positive and professional image and attitude in all relationships with patients, families, peers.

  • Adheres to all Hope House policies and procedures.

  • Maintains absolute confidentiality of all information pertaining to patients, families, and staff and adhere to all HIPAA requirements.

  • Performs all other duties assigned.

Qualifications

  • Experience working with mental health, women’s healthcare, and/or substance use disorders

  • EMR platform experience

  • CPR + First Aid Certification

  • License to work in the state of Georgia

Job Overview

The Counselor Assistant assists in maintaining a safe and structured environment for Hope House, Inc. families. The Counselor Assistant ensures all Policies and Procedures of Hope House, Inc. are followed. The Counselor Assistant reports to the Lead Counselor Assistant.  The counselor assistant plays a critical role in partnering with the Hope House Clinical team in serving participants’ and their families in accordance with the mission and values of Hope House.

Essential Functions

  •  Provides a safe, supportive, and caring environment in which participants and their children will continue their recovery.

  • Supports recovery oriented system of care and is culturally sensitive to clients and employees.

  • Asissts the Medical Department with administering OTC medications as needed during assigned shift.

  • Has comprehensive knowledge of Residents’ Handbook to assist clients and enforce policies and procedures in an encouraging manner.

  • Performs apartment checks.

  • Hourly walks of campus are expected to ensure the safety of participant and their children.

  • Responsible for distributing chore sheets to the participants weekly. Makes note of any participants needing to change their designated chore and reporting those changes to their Counselor/Case Manager.

  • Maintains accurate documentation and knowledge of which participants and/or children are on or off-campus.

  • Maintains daily Pass Log regarding participants and/or children for each shift according to Pass Log Guidelines.

  • Ensures that all participants adhere to the scheduled smoke breaks.

  • Assists in conducting campus wide fire drills and community safety drills.

  • Attend mandatory monthly department meetings.

  • Maintains a regular overnight schedule of checking on participants and their children.  Ensures they are safe and following all safe sleep rules for infants and children. Also ensures that outside access doors are locked and secure overnight.

  • Maintains gate phone and greets every non-employee person that enters the property. Assist them with their needs and ensures proper interaction with residents.

  • Screens and inspects participants’ items when returning from passes, work, and other off-site activities.

  • Screens and inspects participants’ items received from their support person(s) on Drop-off Days.

  • Assess if women are ready to leave on pass by: checking apartments, pass forms are completed, chores are re-assigned and meds are packed for length of time on pass.

  • Compiles incident reports when required by Hope House policies and procedures and/or regulatory agencies.

  • Attends and monitors on-site AA meetings as needed.

  • May provide suggestions to participants relating to personal experience, strength, and hope.

  • Refer participant to her counselor for psychological/emotional issues.

  • Performs all other duties assigned.

Qualifications

  • High School Graduate or GED Equivalency, or enrolled in higher education (Associate Degree or B.A.) in social sciences.

  • Skilled in Microsoft Office Suite and Windows applications.

  • Valid Driver’s License, Clean Driving Record.
